Digital client consent form with signature and PDF
A client signs a consent form with a finger on your phone. You get a ready PDF that can't be changed after the fact.
How it works
- Open the client's card and pick a consent template: for brows, lashes, hair colour or another one.
- Hand the phone to the client. She fills in the questionnaire: allergies, pregnancy and other answers the template asks for.
- The client types her name and signs with a finger on the screen.
- The app checks that every required answer is there and locks the consent: the template text, the answers, the signature and the time.
- Save the PDF or send it to the client. The consent stays on the card and opens without internet.
Versioned templates
The app has consent and questionnaire templates for typical beauty procedures. Each template has a version and an effective date, so you always see exactly which text the client signed.
A signature on the screen
The client reads the text and signs. It works offline, even in a basement studio or during a power cut.
An unalterable PDF
Once signed, the consent is sealed with a SHA-256 hash, the template version and the exact time with its time zone. It can't be edited afterwards. You can save the PDF or send it to the client.
Data under protection
Signed consents count as health data. They are encrypted on the phone, and the backup lives on servers in the EU.
Why a signature can't be “touched up”
After signing, Belanote computes a SHA-256 hash of the template text, the answers, the name, the time and the signature. The template version is stored with the consent, so it is always clear which text the client saw, even if the template is updated later. A signed consent can't be edited.
Template versions
Every change to a template gets a new version and an effective date. Old consents keep the text that was signed. You can download free printable templates on the consent templates page.
Who it helps
Anyone who does procedures where it must be recorded in writing that the client has been informed: brows, lashes, colouring, cosmetology. It is handy when there are many clients and paper forms are hard to keep.
Legal framework
The templates are examples, not legal advice. Requirements for consent depend on the country and the procedure, so have the text checked by a lawyer before you use it.
Questions and answers
Is a signature on the screen legally valid?
That depends on the country and the type of document, so we don't draw that conclusion for you. Belanote locks the text, the answers, the signature and the time so that they cannot be changed. Whether that is enough in your case, ask a lawyer.
Do I need internet to sign a consent?
No. The signature is locked on the phone at once, and syncing happens when a connection appears.
What if the template was updated after signing?
Nothing. The consent keeps the text and the version the client signed.
Where are signed consents stored?
On the phone in an encrypted database, and the backup used for syncing sits on servers in the EU. This is health data, so only the account owner can access it.
