How it works

1. Pick a template: brows, lashes or hair colouring.
2. Download the PDF and print it on A4 or open it on a tablet.
3. The client fills in the health questionnaire: ticks Yes or No and adds details.
4. The client reads the acknowledgement of risks and aftercare, signs and dates it; the practitioner signs too.
5. Keep the filled-in sheet with the client's record. In the Belanote app the same steps happen on the screen and the signature is stored as a PDF.

Consent for brow procedures

For brow tinting, lamination and shaping. Sections: a description of the procedure, a health questionnaire and an acknowledgement of risks and aftercare. 8 questions in all: previous permanent makeup or microblading, allergies, reactions to similar procedures, pregnancy and breastfeeding, inflammation and irritation, medication and photo permission.

Consent for lash procedures

For lash extensions, lamination and tinting. 9 questions in all. It asks separately about contact lenses and eye conditions: conjunctivitis, blepharitis and dry eye.

Consent for hair colouring

For hair colouring and lightening. 9 questions in all. There is a separate item on the patch test before colouring and a question about henna or basma colouring in the last year.

Format, languages, version

Each template is an A4 PDF of up to 70 KB, in Ukrainian and English. All templates are version 1, effective from 9 October 2026. In Belanote every change of text creates a new version with a new effective date.

Who it is for and when it helps

For practitioners who still keep paper questionnaires and for anyone who wants to see what a consent looks like before moving to an electronic one. If the client's health has changed, the questionnaire should be filled in again.
